Evaluations & Certification
A neuromusculoskeletal condition may sometimes affect a person’s ability to perform regular or customary work duties. Phan Wellness Center provides clinical evaluations for patients who need their physical condition and work-related limitations assessed. The condition must fall within the scope of chiropractic practice.
The evaluation includes a review of your symptoms, health history, job duties, and physical demands. An in-person examination may assess movement, strength, mobility, function, and activities that increase your symptoms. All work limitations and documentation must be supported by the clinical findings.
When clinically appropriate, Dr. Gia-Hung Phan, D.C., may provide work-limitation documentation and complete an SDI medical certification. Follow-up examinations may be required before an extension can be considered. Certification is not guaranteed, and final eligibility and approval are determined by the California Employment Development Department.
